外伤性主动脉破裂的病因
对于钝性胸部创伤，通常机制是严重减速伤（例如机动车碰撞或从高处坠落）；患者常有多根肋骨骨折、第一和/或第二肋骨骨折，或其他严重胸部创伤的表现。
穿透性胸部创伤,的伤口通常横穿纵隔（如在乳头或肩胛骨之间穿入）。
外伤性主动脉破裂的病理生理学
主动脉完全破裂通常因大量失血迅速导致死亡，患者通常无法活着从事故现场转运至医院。主动脉部分性破裂伴包裹性破裂通常发生在动脉韧带附近的主动脉峡部（1）（见图），血流通常通过完整的外膜层得以维持。然而，部分破裂也可能会导致局限性纵隔血肿。

外伤性主动脉破裂的症状和体征
外伤性主动脉破裂患者通常有胸痛。
体征包括上肢脉搏减弱、心前区或肩胛间区可闻及刺耳的收缩期杂音、声嘶，以及下肢血流受损的表现（包括下肢脉搏减弱，或下肢血压低于上肢）。
外伤性主动脉破裂的诊断
主动脉成像
如果患者的发病机制或胸部 X 射线检查结果可疑，则应怀疑外伤性主动脉破裂。
有提示作用的胸片结果包括：
纵隔增宽
第一或第二肋骨骨折
主动脉结消失
气管或食道（包括鼻胃管）右移
左主支气管下陷
胸膜或肺尖（通常在左侧）
血胸，气胸或肺挫伤
然而这些提示性X线表现并不会立即出现。此外，没有任何单一或联合的表现具有足够的敏感性或特异性（1）；因此，所有严重减速伤患者均应行主动脉影像学检查，即使体格检查或胸部X线检查无异常发现（2）。
选择何种主动脉影像学检查 因各机构而异。相当准确的检查包括 (3)：
经食管超声心动图不作为初始影像学检查，但如果患者正在接受腹部手术并且血流动力学情况不能用腹部的发现来解释，则可以在术中进行。经食管超声心动图检查的并发症发生率较低，且能检测到CT检查可能遗漏的某些相关损伤（如无名血管损伤）。

外伤性主动脉破裂的治疗
控制血压和心率
血管内支架置入或手术修复
对于钝性创伤性主动脉破裂的患者，应进行液体复苏，但在等待手术修复期间，以及排除其他出血来源后，应开始实施心率血压控制治疗（即降低心率和血压，通常使用β受体阻滞剂，如艾司洛尔）。尽管具体建议有所不同，但目标为心率60-80次/分钟，收缩压≤100 mmHg。
胸主动脉腔内修复术现已成为创伤性主动脉破裂的首选治疗方法（1、2）。在评估和处理其他潜在危及生命的损伤时，修复可延迟进行。
